PUTNAM — Day Kimball Medical Group (DKMG), a division of Day Kimball Healthcare (DKH), was awarded recognition as a Patient-Centered Medical Home™ (PCMH™) from the National Committee for Quality Assurance (NCQA), a private, non-profit organization dedicated to improving health care quality.

The Patient-Centered Medical Home (PCMH) Recognition program highlights medical practices that place patients at the forefront of care, where continuous quality improvement is prioritized.
The PCMH program recognizes primary care providers who have demonstrated effectively coordinated care among all of their patients’ health care providers.
The model benefits patients by delivering accessible services with shorter waiting times for urgent needs, enhanced in-person hours, around-the-clock telephone or electronic access to a member of the team, and alternative methods of communication such as email or telephone care.
This improved access helps them to better understand their treatment options and better manage chronic conditions while creating a partnership with their provider.  
In order to achieve recognition as a PCMH, DKMG’s outpatient locations met a comprehensive range of performance benchmarks for patient care, including effective care planning, coordination, and access to care, as well as measuring performance and quality.
Patient-centered medical homes provide a care model that builds stronger relationships with patients, improves their experience, and reduces unnecessary emergency room visits and hospitalizations.
They aim to ensure that all patients, regardless of factors like socioeconomic stability, access to transportation, or the community in which they live, receive access to the health care they need.
